The iliolumbar ligaments are often the source of lumbosacral pain.
When they are stretched chronically as in an anterolysthesis of L5, they compress the sacroiliac joints. This can cause chronic SI-inflammation and associated pain.
When they are retracted such as in an extension lesion of L5, they also compress the sacroiliac joints. This can cause chronic SI-inflammation and associated pain.
In normal circumstances, they provide stability for the SI-joints.
